• Before scheduling, please confirm your health insurance participation at this link.

Appointment Deposit and No-Show Fee:

• Deposit: A $50 deposit is required to book your appointment. This amount will be applied towards your copay or refunded after insurance processing if no copay is due.

• No-Show Fee: Please note there is a $50 no-show fee. To avoid this, contact us at least 24 hours in advance if you need to cancel or reschedule.
